Care Management

Care Management embraces a broad and comprehensive range of services. These are available to individuals, families, and those responsible for assisting them. It includes:

• An evaluation of the client's physical, functional, and social abilities
• Review financial, legal or medical issues, offer referrals to geriatric specialists to avoid future problems and to conserve assets by developing a cost effective individualized plan
• Screen, arrange and monitor caregivers or other services needed in the home
• Assist with long distance care management
• Assist with all the logistics and moving of a client to retirement community, assisted living or a nursing home
• Alleviation of stress by ensuring delivery of appropriate care
• Provide consumer education and advocacy along with counseling and support
• Provision of confidential and dignified service
